What is Form LMS?
Form LMS is a learning management system that gives you access to training on any device, at any time. Form lets you complete digital courses online, join virtual sessions remotely, or attend face-to-face training in person.
Your role: Learner
Getting started
To create an account on Form, you must be invited by your organisation or their training provider. This can be through an email invitation or a self-registration link.
Accept an invite
Accept your invitation and set up your account to access Form LMS.
Use a self-registration link
If your organisation uses self-registration, use a link to create your own account and select your organisation unit.
Log in
Once you have created your account, access Form using your email address and password.
Navigate Form LMS as a learner
Understand the learner dashboard and find your courses, completions and available training.
Accessing your learning
My Courses
View and launch courses you are enrolled on. Track your progress through modules and see your completion status.
My Completions
Access and download certificates for courses you have completed.
Browse Courses
Discover courses available for self-enrolment. You can also request access to courses with conditional enrolment.
Event sessions
Book a session as a learner
Book yourself onto available event sessions for courses that include face-to-face or virtual training.
Cancel a session as a learner
Cancel your booking if you can no longer attend a scheduled session.
Account management
Reset your password
Change your password when logged in or reset it if you have forgotten it.
Enable two-factor authentication
Add an extra layer of security to your account using a two-factor authentication app.
