Feedback forms let you collect course feedback directly from your learners. You can create new templates or use the pre-set feedback form.
✅ Before you start: You need the Site admin or Admin role.
Enable feedback forms
- Go to Setup → Site settings → Feedback forms.
- Click the toggle in the top left corner that reads Feedback forms disabled.
- In the modal that appears, click Enable.
Feedback forms are now active on your workspace.
Create a feedback form template
- Click Create new template and enter a name for the form.
- Click Add field. A configuration box appears.
- Enter your question under Field label. This displays above where learners leave their response.
- Choose the field type. This determines how the learner answers:
- Text - learners answer using free text.
- Checkbox - learners answer by ticking or leaving a box unticked.
- Single choice - learners select one pre-defined option. Type options under the Options field, with each option on a new line. Select Add 'Other' option to allow a free text answer.
- Multi choice - learners select multiple pre-defined options. Type options under the Options field, with each option on a new line. Select Add 'Other' option to allow a free text answer.
- Add a description in the Help text area to explain the field's purpose.
- To make the field mandatory, tick Field is mandatory.
- Click Add field to form.
- Repeat until you have built the desired feedback form.
- Click Save feedback form.
Your new feedback form template is now saved and ready to assign to courses.
Set a feedback form as the workspace default
You can set a feedback form template as the default used by courses configured to use the workspace default.
- Find the feedback form and click the three-dot menu.
- Select Make default.
Courses set to use the workspace default feedback form now use this template.
